***IMPORTANT*** The following forms have been incorporated into the registration form to be signed electronically this year. There’s no need to print and sign them. The forms below are just for your viewing pleasure.

Megan’s Law Form — Required to print, sign and submit to the respective schools if you are coaching an event this year. Each school will require one form. So if you have a child in middle school and a child in high school, you will have to submit one to the middle school for one child and another one for the child in high school.

Registration fee of $125 is due on September 17th. If you wish to pay via checks, please make checks payable to “Castro Valley Science” and note the name of the school and the name(s) of your child(ren).
